What kinds of changes are recorded?

Does it show who made each change?

Yes. Every event carries the name of the person who made the change and the time it happened.

Can any staff member read any participant's history?

No. History follows the same access rules as the participant record, so users only see timelines for participants they are allowed to work with.

Is history kept when a participant is deactivated?

Yes. The record and its full timeline are preserved, and the deactivation itself appears as an event, as does any later reactivation.

Is this the same as the Audit Centre?

They do different jobs. Client history is the per-participant timeline of changes; the Audit Centre organises your evidence against the NDIS Practice Standards for audits.
