Structured repeating entries

Use repeating Table fields when one entry needs several rows, and configure fixed Choice options or participant-specific choices where the record needs them.

Assigned to the participant

Publish a Chart template, assign it to the participant who needs it, choose when it takes effect, and keep other participants' Charts out of that record.

Entries and period PDFs

Managers can review Chart Entries in a register and download a PDF for a chosen period, with every entry shown against the configuration used when it was recorded.

Starter templates can support structured operational observations such as behaviour, food and fluid, sleep and medication-related records. You decide the fields, choices, access and review process that fit your organisation.

Charts are configurable operational software. They do not provide clinical advice, prescribe care, determine medication requirements or guarantee NDIS compliance.

Historical entries stay understandable

Configuration changes take effect from a date you choose. Previous entries retain the fields and choices that applied at the time, so a later edit does not rewrite the record.

Frequently asked questions.

What is the difference between a Report and a Chart?

A Report records an event, note or assessment. A Chart is for the same structured observation recorded repeatedly over time for one participant, with its own entry history and date-range PDF.

Can a Chart be created for only one participant?

Yes. Assign a published Chart to one participant and configure client-specific Choice options where needed. The Chart then belongs to that participant's record.

What happens when a Chart changes?

Choose when the new configuration takes effect. New entries use the effective configuration, while historical entries keep the questions and choices that applied when they were recorded.

Can Charts use repeating rows?

Yes. A Table field supports structured repeating rows, which is useful when one observation period needs several line items rather than one long text box.

Can we use a Chart for medication-related records?

You can configure an operational Chart for medication-related recording. BondiByte does not prescribe medication, determine clinical requirements, replace clinical systems or guarantee compliance. Your organisation remains responsible for clinical governance, policy, review and escalation.

Can frontline staff complete Charts in the mobile app?

The SaaS and staff-portal APIs support client-first Chart discovery and submission. The matching native mobile build has not yet been verified as a public App Store or Google Play release, so confirm current mobile availability with BondiByte during onboarding.
